Cost:
Medicare and Blue Sheild of California: Dr Rowe is a credentialed provider by Medicare and Living Healthy Physical Therapy is a credentialed provider for Blue Shield of California. Living Healthy Physical Therapy submits claims to Medicare or Blue Shield and any supplementary insurance you have. You may be responsible for a small deductible and possibly a copay depending on your supplementary insurance. We are happy to find out for you approximately how much (if anything) you will be responsible for. Insurance: Living Healthy Physical Therapy is currently an out of network provider for other insurances. This means that you will be required to pay a reasonable flat cash rate at the time of each service. You will receive a very detailed receipt that will provide all the information that you need from us in order to get reimbursed by your insurance company or to have your payment applied to your deductible.
Your first session will be a 60–90-minute detailed evaluation where you will take away a comprehensive home program and self-treatment regimen (Cash Pay Rate: $150 ($200 in home). Your follow up sessions, if and as needed, will be 60 minutes long (Cash Pay Rate: $125 ($150 in home). They will involve comprehensive therapeutic exercise, manual therapy, equipment and activity recommendations as well as follow up emails with program adjustments.

Do I need a referral: In the State of California, a physician referral for physical therapy services is not required initially, however, most insurances require a doctor's order for reimbursement. Should you choose self-referral, you may continue to receive direct physical therapy treatment services for a period of up to 45 calendar days or 12 visits, whichever occurs first. After that time, a physical therapist may continue to provide treatment only after receiving a dated physician signature on the physical therapist’s plan of care indicating approval of the physical therapist’s plan of care and that an in-person patient examination and evaluation was conducted by the physician, surgeon or podiatrist. (Medicare rules differ slightly but we are happy to discuss this with you)
